Individualized Care
We support individuals in being more independent while meeting their daily living needs. Our homemaker/personal care (HPC) increases the individual’s independence within his/her home and community and helps the individual meet daily living needs. Examples of support include:

HOMEMAKER PERSONAL CARE
Assisting with bathing and other daily grooming tasks, medical assistance such as first aid and medication administration or reminders to take medication.
​
TRANSPORTATION
Provided to assist individuals with accessing community activities such as grocery shopping, going on community outings, securing medical services, and to and from work.
​
COMMUNITY
We encourage self advocacy and provide assistance with accessing the community.
